New and Improved
Dental and Vision Plans!
- Lower premiums, better
coverage than previous plans!
- The plans allow you
to use the dentist or eye care provider of your choice and to obtain
the services you and your provider agree upon. (There are no dental
or vision networks.)
- No waiting periods
before certain procedures are covered.
- No excluded procedures
except cosmetic teeth bleaching, orthodontia, and eye surgery.
- No requirements for
pre-approval of a procedure.
- No maximum fee schedule
for each procedure. Your chosen provider’s normal fees are allowable
expenses.
Preferred
Dental & Vision Plan:
You and your covered dependents
each have $1,250 in benefits for dental and/or vision care.
Your benefits may include any combination of dental and vision expenses.
The plan reimburses you:
- 100% of the first $225
of eligible dental and/or vision expenses, then
- 0% of the next $50
of eligible expenses, then
- 80% of the next $100
of eligible expenses, then
- 50% of the remaining
eligible expenses to a maximum benefit of $1,250 per year per person.
- The plan covers all
dental services except orthodontia and cosmetic teeth bleaching procedures.
- Vision covers exams,
frames, lenses, and contacts. (No surgery)
You and your covered dependents
each have $750 in benefits for dental care. The plan reimburses
you:
- 100% of the first $175
of eligible dental expenses, then
- 0% of the next $50
of eligible expenses, then
- 80% of the next $100
of eligible expenses, then
- 50% of the remaining
eligible expenses to a maximum benefit of $750 per year, per person.
- This plan does not
cover orthodontia benefits.
How Direct Reimbursement is
Used and How Reimbursements are Made
You simply present your ID
card to your chosen provider and you receive the stated benefits mentioned
above guaranteed! Either your dentist or you submit the invoice
and a claim form via fax or mail and the reimbursement will be issued
within 5 business days of receipt.
